HIPAA Notice of Privacy Practices

I am required by law to maintain the highest standards of privacy and security to protect your Health Information (PHI) and to provide you with this notice. If, for any reason, your unsecured PHI is compromised, you will be notified immediately.

Privacy Policy - the basics

I will maintain the confidentiality of information throughout the counseling relationship as mandated by federal and state laws and all ethical standards. All communication between you and me will be held in the strictest confidence unless you provide me with written permission to release information about your counseling or treatment. There are, however, several specific exceptions to this confidential agreement, which will be outlined below.

Privacy Policy

According to Washington and Federal law, I am required and permitted to disclose your information under the following conditions:

Threat of Harm to self or others, which includes those identifiable by their association with a specific location or entity, including grave disability, suicidal, or homicidal harm.
Known for suspected child abuse or neglect
Known perspective on elder abuse or neglect
Any information subpoenaed from a court of law, a court order, or other mandated state or federal laws.
If the Client is a minor (under age 13), parents or guardians will be able to access their healthcare information
Written consent from Client Release of Information (ROI)
If a Legal investigation or complaint or investigation is ongoing, such as a civil suit.
If Client communicates any information involving a threat to you or others, I may be required to take immediate action to protect you and others from harm.
Diagnosis or other relevant clinical information/documentation required by your health insurance company, which has agreed to assist in the fee payment of your care.

In Addition, as a Therapist who uses Telehealth systems, I will abide by all HIPAA-compliant confidentiality and privacy requirements related to Telehealth, utilizing a confidential, live video/audio system (Zoom). Nothing will be recorded on Zoom or any other mobile app, phone call, or e-mail system that I utilize. All platforms used are virus- and password-protected.
